Hello everyone,

I am ready for bed but wanted to post quick with a update from my appointment today.

I went to the dermatologist with the understanding that a biopsy would be done to see why my skin & nails are changing so much and my hair is literally falling out of my head at rapid rates. I was also there for my annual skin cancer check.

Well, I do not have any suspicious moles or cancers on my skin so that is good. I have a pre-cancerous area above my lip that they freeze when it flares up and so they did that today and I am a little sore but no big deal.

As far as the visible changes to the skin on my hands, around my cuticles and palms too he prescribed a prescription lotion called Lachydrin Lotion to be filled at my pharmacy. He said put it on in the morning and at night with gloves to try and reduce the issues.

Now as for the biopsy he told me that he will not do a biopsy as they really do not show much.:eek::eek::eek: OK, so I proceed to politely challenge him and tell him about The Lupus Site, my friends here, the stories I have read about skin biopsy's that clearly show a lot and are very helpful to aid in diagnosis or at least help to paint a clearer picture. He did not like that very much and reminded me that in fact he was the M.D., not me.

At this point I was really mad because my Rheumatologist wrote a letter requesting that a biopsy be done and results of the pathology be sent to his office as well so I told the Dermatologist that clearly biopsy's do show things or my Rheumy would not have requested it be done. His reply to me, "I am the skin guy and it wont show anything".

He wrote a script for the lotion and for bloodwork. He is checking my ANA, Ferritin and TSH 3 and 4..........told me to come back in a month. Then he suggested it is probably hormonal and I told him that is impossible as I hit menopause years ago! I asked him if he reads his charts before he opens his mouth.:lol::lol::lol:

So the moral of the story here is I pissed off the doctor today because he was just a pompous know it all who in my humble opinion did not do his job and disregarded the requests of my Rheumy. When I got home I called my Rheumy and left a message for him to call me back and discuss this. I know that he will not be happy and the war between doctors will begin.:rolleyes:

Is anything ever easy in life? The older I get the more I challenge doctors like this one and it really is exhausting. I had the blood work done yet I am left to wonder if a biopsy would have provided some answers before I have to put my head into a wig.:mad::mad::mad:

I am done venting, feel better but tired. I am going to bed now but hope you all had a pain free day.:wink2:

I'm wondering if you could get to Penn Uni ? One of the US top CTD dermy specialists is there, Dr Victoria Werth.

I was once prescribed some alphahydroxy acid stuff maybe similar to Lac- hydrin I think, which is lactic acid I believe not by my eventual specialist, just a GP trying to be helpful. I reacted rather badly to it with redness stinging as you can read about, but maybe you will find it brings you soothing relief. Increased photosensitivity is frequently mentioned as a risk with this sort of product.

:grhug: :eek: What another weird and horrible ordeal!

Hope you got a nice sleep and I'm sure you'll be more than ready to put the medical world to rights!

You still seem to be able to make to laugh (even with this post) even when reliving terrible ordeals...I think its your humour thats keeps you strong:wink2:. I know you won't take offence from this comment!:hehe:

Hi Karol..sorry you had to deal with a bumptious know it all.. but well done for having none of it!! You are better informed than him and should not have to apologise for that fact..indeed he should be ashamed, or at least go look into it more if it isn't his area of expertise...what an idiot.:(.... at least your Rheumy will not refer lupus patients to him again, so although it is a disappointment you have done some groundbreaking research for your Rheumy;)..hopefully your Rheumy will now re-refer you to a more educated (or at least willing to be educated....) Dermy... sleep well, talk again soon.

Well Terri apparently he thinks my thyroid is bad or iron levels are low??? Then again he also thought it could be hormonal............HE IS AN IDIOT!!! As for the ANA, not sure why he ran it. He has copies of past blood work clearly showing a high ANA titer on more then one occasion so I am not sure, possibly to waste the Insurance companies money.:lol::lol::lol:

Thank you all for your replies as it helps me feel validated and thank god for the information I have learned from all of you. I would not have been able to stand up to this pompous idiot had I not been a member here with a lot more knowledge now then when this journey began for me.

I will let you know how it goes after my Rheumy gets his sorry butt on the phone, and trust me he will.:wink2: He is a no nonsense guy who by the way was rated one of the Top Docs for 2009.

Clare I have heard mention of Dr. Werth out of U of Penn and I think I will call and schedule with her. It may be a 6 month wait yet worth it for sure.

Lesley I would never take offense to anything from you. Your actually right in what you said!!!:rotfl:

Hi Karol, sorry you had such an awful appt. :eek: I can't believe a dermy thinks skin biopsys don't show anything!!! :eek: Maybe he doesn't know how to do them!! :lol:

I can just imagine you challenging the doc, you do make me chuckle!! I wish you well in getting this situation sorted out.

Hi karol

well done for standing up for yourself. Grrrrr these so called experts. Did your rheumy refer you to him for a biopsy.

Such a shame he wouldnt do it my rheumey sent me to see dermy I had a skin biopsy and it showed up sclerderma. I also was sent for a CT scan.

Hope you get some meds sorted to help you with your hair and skin

Bless your heart. I hope you never go back to him again. Maybe your Rheumy can refer you to someone else who is more competent in the medical field. Kudos for you sticking up for yourself. I hope things get better and you get some answers soon!!

Hi Dixy,

No, this Dermatologist has been my doctor for years now so my Rheumy felt safe to request a biopsy be done.

My Rheumy did return my call this morning and I explained what happened. He thanked me for sharing my experience and then told me he was calling the Dermatologist to discuss this with him.

O Boy if I could be a fly on the wall for that conversation.:lol::lol::lol::lol::lol:

By the way, Clare is right. This lotion does burn, sting and you must be very careful while out in the sunlight. So I guess if I have to be outside then the lotion will not go on my hands.

I got the Generic brand and it is called Ammonium 12%.

Sorry you had such a ridiculous appt Karol! Just when we think we have heard it all someone surprises us :rolleyes::rolleyes: and not in a good way :hug:

Here's to your Rheumy resolving this and giving the guy a good kick up the backside - he sure deserves it.

I just wonder whether all these so-called professionals have any idea that the revolution is at hand and that all the worms are beginning to turn............?!?!?! Let's hope that our posts at least make their ears burn !

The sad truth is that we can all recognize/identify with Karol's bad experience - and its not just because she tells her story so well !

Hi Karol,
I am amazed a doctor had the audacity to claim his degree!!!! Mercy sakes, maybe he needs to go talk to his medical school profs and get their spin on what skin damage can be done by sle! My oncologist has many times said that sle patients know medicine more than most of his total practice combined! I informed him it was survival to be able to know what this disease is and does!

Sorry you are on the east coast, I have a couple great derm docs out here in Seattle that I could refer you to! When my rheumy req biopsies, they were done, no problem.

Yup, would LOVE to be a fly on that wall for the tele call from one specialist to another! Bet it was a doozie of a talk! Pompous jerks get known about around the water cooler too!

Thanks Sally. Well Monday my husband flys out to Washington on business. He starts in the south and will work his way towards Seattle as he has done plenty of times before.

Maybe he can stick those top docs in his suitcase and bring em back here to the arm pit of the world. Do you think they would be receptive to the idea?:rotfl:

Sorry, Karol, NO WAY would they be allowed by their patients to leave here!
Better you get on a plane and head out here. I know, I know, I know.....I don't fly any more. Every time I got on a plane, I ended up with a lung infection and was on antibiotics for a month or more.
Safe travels for your husband in the coming week.

Hi Karol,
Send an umbrella with your hubby for his time here. We are having heavy rain squalls the last couple days. Hopefully by the time he gets here later this week we will be done with them, but there is no guarantee.
